- ItemOpen AccessDevelopments in the production of economics PhDs at four research-intensive universities in South Africa(2017) de Jager, Phillip; Frick, Liezel; van der Spuy, PieterAbstractThere is a national drive to increase PhD production, yet we know little about how this imperative takes shape within different disciplines. We therefore set out to explore recent developments and the current status of the PhD in economics at four South African research-intensive universities. A data set of all economics PhDs produced in these commerce faculties during the period 2008–2014 was analysed to determine whether the departments of economics responded to the call for increased doctoral production, and the role the PhD by publication might have played in the process. How an increase in quantity might influence doctoral education in the respective academic departments was also considered by supplementing the quantitative data with perspectives from heads of department at the four institutions. The notable increase in doctoral production over the time period studied shows that national and international trends have influenced doctoral education in economics departments within South African research-intensive universities. Increased usage of the PhD by publication has implications for policy and pedagogical practice within these departments, especially as there seems to be limited available supervisory capacity. Other changes in departmental practices, such as the entrenchment of a research culture and the promotion of collaborative research amongst students and staff, also contributed to maintain quality in doctoral education.
- ItemRestrictedNovice occupational therapists: Navigating complex practice contexts in South Africa(2019-01-30) Van Stormbroek, Kirsty; Buchanan, HelenBackground/aim: The transition from student to occupational therapy practitioner is challenging. In South Africa, this transition is undertaken in rural and underserved areas, as graduate health professionals are deployed by the government for a year of compulsory Community Service. This study set out to establish the characteristics of these practice settings, the resources available for occupational therapy services, the availability and quality of supervision, and participants’ perceived ability to communicate with their patients and negotiate cultural differences. Methods: A cross‐sectional survey design was utilised and a questionnaire was sent to all occupational therapists completing Community Service in 2013 (N = 240). Data were analysed using Stata 12 and IBM SPSS Statistics 21.0. Frequencies and percentages were calculated for categorical variables and associations tested with Odds Ratios and the Pearson's Chi square test. Responses to open‐ended questions were post‐coded. Results: A 44.3% (n = 103) response rate was achieved. Practice settings often provided few resources. Although most participants had supervisors (89.6%), many did not find supervision satisfactory (65.9%). Communication difficulties featured strongly (73.9%), but the majority of participants felt they possessed basic cultural competence. Conclusion: Participants worked within complex practice settings that were frequently resource‐restricted with less than satisfactory supervision. Practice required cultural competence and an ability to work across language barriers. Undergraduate curricula need to be tailored to equip new graduates to navigate these contextual realities. Furthermore, human resourcing strategies need to be evaluated and effective supervision and support structures need to be developed.
- ItemOpen AccessSurgical trainee supervision during non-trauma emergency laparotomy in Rwanda and South Africa(2021) Pohl, Linda M; Chu, KathrynObjective: The primary objective was to describe the level of surgical trainee autonomy during non-trauma emergency laparotomy (NTEL) operations in Rwanda and South Africa. The secondary objective was to identify potential associations between trainee autonomy, and patient mortality and reoperation. Design, Setting, and Participants: This was a prospective, observational study of NTEL operations at three teaching hospitals in South Africa and Rwanda over a oneyear period from September 1, 2017 – August 31, 2018. A total of 543 operations on adults over the age of 18 years who underwent NTEL performed by the acute care and general surgery services were included. Results: surgical trainees led three quarters of NTEL operations, and of these, 72% were performed autonomously in Rwanda and South Africa. Trainees were less likely to perform the operations autonomously for patients who were: age ≥ 60 years, had ASA classification ≥ III, had cancer or TB. Notably, trainee autonomy was not significantly associated with reoperation or mortality. Conclusions: trainees were able to gain autonomous surgical experience without impacting mortality or reoperation outcomes, while still providing surgical support in a high-demand setting. More in-depth studies to understand the association of high trainee autonomy with surgical competency and patient safety is needed.
