- ItemOpen AccessOrganisational culture and trust as influences over the implementation of equity-oriented policy in two South African case study hospitals(2017) Erasmus, Ermin; Gilson, Lucy; Govender, Veloshnee; Nkosi, MoremiBACKGROUND: This paper uses the concepts of organisational culture and organisational trust to explore the implementation of equity-oriented policies - the Uniform Patient Fee Schedule (UPFS) and Patients' Rights Charter (PRC) - in two South African district hospitals. It contributes to the small literatures on organisational culture and trust in low- and middle-income country health systems, and broader work on health systems' people-centeredness and "software". METHODS: The research entailed semi-structured interviews (Hospital A n = 115, Hospital B n = 80) with provincial, regional, district and hospital managers, as well as clinical and non-clinical hospital staff, hospital board members, and patients; observations of policy implementation, organisational functioning, staff interactions and patient-provider interactions; and structured surveys operationalising the Competing Values Framework for measuring organisational culture (Hospital A n = 155, Hospital B n = 77) and Organisational Trust Inventory (Hospital A n = 185, Hospital B n = 92) for assessing staff-manager trust. RESULTS: Regarding the UPFS, the hospitals' implementation approaches were similar in that both primarily understood it to be about revenue generation, granting fee exemptions was not a major focus, and considerable activity, facility management support, and provincial support was mobilised behind the UPFS. The hospitals' PRC paths diverged quite significantly, as Hospital A was more explicit in communicating and implementing the PRC, while the policy also enjoyed stronger managerial support in Hospital A than Hospital B. Beneath these experiences lie differences in how people's values, decisions and relationships influence health system functioning and in how the nature of policies, culture, trust and power dynamics can combine to create enabling or disabling micro-level implementation environments. CONCLUSIONS: Achieving equity in practice requires managers to take account of "unseen" but important factors such as organisational culture and trust, which are key aspects of the organisational context that can profoundly influence policies. In addition to implementation "hardware" such as putting in place necessary staff and resources, it emphasises "software" implementation tasks such as relationship management and the negotiation of values, where equity-oriented policies might be interpreted as challenging health workers' status and values, and paying careful attention to how policies are practically framed and translated into practice, to ensure key equity aspects are not neglected.

- ItemOpen AccessPython based FPGA design-flow(2016) New, Wesley; Inggs, Michael; Winberg, SimonThis dissertation undertakes to establish the feasibility of using MyHDL as a basis on which to develop an FPGA-based DSP tool-ow to target CASPER hardware. MyHDL is an open-source package which enables Python to be used as a hardware definition and verification language. As Python is a high-level language, hardware designers can use it to model and simulate designs, without needing detailed knowledge of the underlying hardware. MyHDL has the ability to convert designs to Verilog or VHDL allowing it to integrate into the more traditional design-ow. The CASPER tool- ow exhibits limitations such as design environment instability and high licensing fees. These shortcomings are addressed by MyHDL. To enable CASPER to take advantage of its powerful features, MyHDL is incorporated into a next generation tool-ow which enables high-level designs to be fully simulated and implemented on the CASPER hardware architectures.
- ItemOpen AccessRNAseq analysis of heart tissue from mice treated with atenolol and isoproterenol reveals a reciprocal transcriptional response(2016) Bergmann, SvenAbstract Background The transcriptional response to many widely used drugs and its modulation by genetic variability is poorly understood. Here we present an analysis of RNAseq profiles from heart tissue of 18 inbred mouse strains treated with the β-blocker atenolol (ATE) and the β-agonist isoproterenol (ISO). Results Differential expression analyses revealed a large set of genes responding to ISO (n = 1770 at FDR = 0.0001) and a comparatively small one responding to ATE (n = 23 at FDR = 0.0001). At a less stringent definition of differential expression, the transcriptional responses to these two antagonistic drugs are reciprocal for many genes, with an overall anti-correlation of r = −0.3. This trend is also observed at the level of most individual strains even though the power to detect differential expression is significantly reduced. The inversely expressed gene sets are enriched with genes annotated for heart-related functions. Modular analysis revealed gene sets that exhibit coherent transcription profiles across some strains and/or treatments. Correlations between these modules and a broad spectrum of cardiovascular traits are stronger than expected by chance. This provides evidence for the overall importance of transcriptional regulation for these organismal responses and explicits links between co-expressed genes and the traits they are associated with. Gene set enrichment analysis of differentially expressed groups of genes pointed to pathways related to heart development and functionality. Conclusions Our study provides new insights into the transcriptional response of the heart to perturbations of the β-adrenergic system, implicating several new genes that had not been associated to this system previously.
- ItemOpen AccessText entry, analysis and correction help : assisting the disabled computer user with data entry(1990) Hirson, Guy; Hanmer, Lyn Avril; Boonzaier, DavidIt was suggested several decades ago that computers would be the single biggest step forward in integrating people with physical disabilities into "normal" society. At that stage, much work was done in writing software and designing hardware that allowed computer operators with disabilities to use packages effectively, in certain cases as efficiently as people without disabilities. Since those days, judging by the lack of references on this subject the interest in dealing with disabled people has waned. It is only very recently that the spotlight has been focused on these potentially very productive persons. Unfortunately, the backlog is large and most existing applications software offers little or no support for users with disabilities. In this thesis, I have examined some of the hardware and software limitations of current desktop computer technology, focusing on the IBM PC and compatibles. I have also written a computer program that attempts to relieve some of the difficulties faced by a limited number of disabled users. In evaluating the results, I considered it important to relate the ensuing data with the real problems faced by a far wider spectrum of users than I attempted to cater for with the program and to suggest ways in which software products could be made to have wider applicability in the future.
