- ItemOpen AccessComparison of six methods to estimate adherence in an ART-naïve cohort in a resource-poor setting: which best predicts virological and resistance outcomes?(2017) Orrell, Catherine; Cohen, Karen; Leisegang, Rory; Bangsberg, David R; Wood, Robin; Maartens, GaryBACKGROUND: Incomplete adherence to antiretroviral therapy (ART) results in virologic failure and resistance. It remains unclear which adherence measure best predicts these outcomes. We compared six patient-reported and objective adherence measures in one ART-naïve cohort in South Africa. METHODS: We recruited 230 participants from a community ART clinic and prospectively collected demographic data, CD4 count and HIV-RNA at weeks 0, 16 and 48. We quantified adherence using 3-day self-report (SR), clinic-based pill count (CPC), average adherence by pharmacy refill (PR-average), calculation of medication-free days (PR-gaps), efavirenz therapeutic drug monitoring (TDM) and an electronic adherence monitoring device (EAMD). Associations between adherence measures and virologic and genotypic outcomes were modelled using logistic regression, with the area under the curve (AUC) from the receiver operator characteristic (ROC) analyses derived to assess performance of adherence measures in predicting outcomes. RESULTS: At week 48 median (IQR) adherence was: SR 100% (100-100), CPC 100% (95-107), PR-average 103% (95-105), PR-gaps 100% (95-100) and EAMD 86% (59-94), and efavirenz concentrations were therapeutic (>1 mg/L) in 92%. EAMD, PR-average, PR-gaps and CPC best predicted virological outcome at week 48 with AUC ROC of 0.73 (95% CI 0.61-0.83), 0.73 (95% CI 0.61-0.85), 0.72 (95% CI 0.59-0.84) and 0.64 (95% CI 0.52-0.76) respectively. EAMD, PR-gaps and PR-average were highly predictive of detection of resistance mutations at week 48, with AUC ROC of 0.92 (95% CI 0.87-0.97), 0.86 (0.67-1.0) and 0.83 (95% CI 0.65-1.0) respectively. SR and TDM were poorly predictive of outcomes at week 48. CONCLUSION: EAMD and both PR measures predicted resistance and virological failure similarly. Pharmacy refill data is a pragmatic adherence measure in resource-limited settings where electronic monitoring is unavailable. Trial registration The trial was retrospectively registered in the Pan African Clinical Trials Registry, number PACTR201311000641402, on the 13 Sep 2013 ( www.pactr.org ). The first participant was enrolled on the 12th July 2012. The last patient last visit (week 48) was 15 April 2014.
- ItemOpen AccessEverolimus for treatment of tuberous sclerosis complex-associated neuropsychiatric disorders(2017) Krueger, Darcy A; Sahin, MustafaAbstract Objective: To evaluate if short‐term treatment with everolimus was safe and could improve neurocognition and behavior in children with TSC. Methods: This was a prospective, double‐blind randomized, placebo‐controlled two‐center phase II study. Participants diagnosed with TSC and age 6–21 years were treated with 4.5 mg/m2 per day of oral everolimus (n = 32) or matching placebo (n = 15) taken once daily for 6 months. For efficacy, a comprehensive neurocognitive and behavioral evaluation battery was performed at baseline, 3 months, and 6 months. For safety, adverse events recorded continuously via patient diary were categorized and graded per NCI Common Toxicity Criteria for Adverse Events, version 3.0 (CTCAE 3.0). Analyses were performed on the intention‐to‐treat population (n = 47). Results: Nearly all assessment measures failed to demonstrate significant differences between the two groups at the end of 6 months. Only one measure each of executive function (Cambridge Neuropsychological Test Automated Battery Stockings of Cambridge) favoring placebo (P = 0.025) and social cognition (Social Responsiveness Scale Social Cognition Subscale) favoring everolimus (P = 0.011) was observed. A total of 473 adverse events (AE) were reported. The average number of total AE per subject was similar for both placebo and everolimus. Most were mild or moderate in severity and serious AE were rare. Interpretation While safe, oral everolimus administered once daily for 6 months did not significantly improve neurocognitive functioning or behavior in children with TSC.
- ItemOpen AccessIndian Ocean Dipole and El Niño/Southern Oscillation impacts on regional chlorophyll anomalies in the Indian Ocean(2013) Currie, J C; Lengaigne, M; Vialard, J; Kaplan, D M; Aumont, O; Naqvi, S W A; Maury, OThe Indian Ocean Dipole (IOD) and the El Niño/Southern Oscillation (ENSO) are independent climate modes, which frequently co-occur, driving significant interannual changes within the Indian Ocean. We use a four-decade hindcast from a coupled biophysical ocean general circulation model, to disentangle patterns of chlorophyll anomalies driven by these two climate modes. Comparisons with remotely sensed records show that the simulation competently reproduces the chlorophyll seasonal cycle, as well as open-ocean anomalies during the 1997/1998 ENSO and IOD event. Results suggest that anomalous surface and euphotic-layer chlorophyll blooms in the eastern equatorial Indian Ocean in fall, and southern Bay of Bengal in winter, are primarily related to IOD forcing. A negative influence of IOD on chlorophyll concentrations is shown in a region around the southern tip of India in fall. IOD also depresses depth-integrated chlorophyll in the 5–10° S thermocline ridge region, yet the signal is negligible in surface chlorophyll. The only investigated region where ENSO has a greater influence on chlorophyll than does IOD, is in the Somalia upwelling region, where it causes a decrease in fall and winter chlorophyll by reducing local upwelling winds. Yet unlike most other regions examined, the combined explanatory power of IOD and ENSO in predicting depth-integrated chlorophyll anomalies is relatively low in this region, suggestive that other drivers are important there. We show that the chlorophyll impact of climate indices is frequently asymmetric, with a general tendency for larger positive than negative chlorophyll anomalies. Our results suggest that ENSO and IOD cause significant and predictable regional re-organisation of chlorophyll via their influence on near-surface oceanography. Resolving the details of these effects should improve our understanding, and eventually gain predictability, of interannual changes in Indian Ocean productivity, fisheries, ecosystems and carbon budgets
- ItemOpen AccessMultisegment rupture in the 11 July 1889 Chilik earthquake ( M w 8.0-8.3), Kazakh Tien Shan, interpreted from remote sensing, field survey, and paleoseismic trenching: THE M w 8.0-8.3 CHILIK EARTHQUAKE(2016) Abdrakhmatov, K E; Walker, R T; Campbell, G E; Carr, A S; Elliott, A; Hillemann, C; Hollingsworth, J; Landgraf, A; Mackenzie, D; Mukambayev, A; Rizza, M; Sloan, R AThe 11 July 1889 Chilik earthquake (M-w 8.0-8.3) forms part of a remarkable sequence of large earthquakes in the late nineteenth and early twentieth centuries in the northern Tien Shan. Despite its importance, the source of the 1889 earthquake remains unknown, though the macroseismic epicenter is sited in the Chilik valley, similar to 100 km southeast of Almaty, Kazakhstan (similar to 2 million population). Several short fault segments that have been inferred to have ruptured in 1889 are too short on their own to account for the estimated magnitude. In this paper we perform detailed surveying and trenching of the similar to 30 km long Saty fault, one of the previously inferred sources, and find that it was formed in a single earthquake within the last 700 years, involving surface slip of up to 10 m. The scarp-forming event, likely to be the 1889 earthquake, was the only surface-rupturing event for at least 5000 years and potentially for much longer. From satellite imagery we extend the mapped length of fresh scarps within the 1889 epicentral zone to a total of similar to 175 km, which we also suggest as candidate ruptures from the 1889 earthquake. The 175 km of rupture involves conjugate oblique left-lateral and right-lateral slip on three separate faults, with step overs of several kilometers between them. All three faults were essentially invisible in the Holocene geomorphology prior to the last slip. The recurrence interval between large earthquakes on any of these faults, and presumably on other faults of the Tien Shan, may be longer than the timescale over which the landscape is reset, providing a challenge for delineating sources of future hazard.
- ItemOpen AccessNo Evidence for Extensions to the Standard Cosmological Model(2017) Heavens, Alan; FANTAYE, YABEBAL; Sellentin, Elena; Eggers, Hans; Hosenie, Zafiirah; Kroon, Steve; Mootoovaloo, ArrykrishnaWe compute the Bayesian Evidence for models considered in the main analysis of Planck cosmic microwave background data. By utilising carefully-defined nearest-neighbour distances in parameter space, we reuse the Monte Carlo Markov Chains already produced for parameter inference to compute Bayes factors $B$ for many different model-dataset combinations. Standard 6-parameter flat $Lambda$CDM model is favoured over all other models considered, with curvature being mildly favoured only when CMB lensing is not included. Many alternative models are strongly disfavoured by the data, including primordial correlated isocurvature models ($ln B-7.8$), non-zero scalar-to-tensor ratio ($ln B-4.3$), running of the spectral index ($ln B = -4.7$), curvature ($ln B-3.6$), non-standard numbers of neutrinos ($ln B-3.1$), non-standard neutrino masses ($ln B-3.2$), non-standard lensing potential ($ln B-4.6$), evolving dark energy ($ln B-3.2$), sterile neutrinos ($ln B-6.9$), and extra sterile neutrinos with a non-zero scalar-to-tensor ratio ($ln B-10.8$). Other models are less strongly disfavoured with respect to flat $Lambda$CDM. As with all analyses based on Bayesian Evidence, the final numbers depend on the widths of the parameter priors. We adopt the priors used in the Planck analysis, while performing a prior sensitivity analysis. Our quantitative conclusion is that extensions beyond the standard cosmological model are disfavoured by Planck data. Only when newer Hubble constant measurements are included does $Lambda$CDM become disfavoured, and only mildly, compared with a dynamical dark energy model ($ln Bsim +2$).
- ItemOpen AccessOverdensities of SMGs around WISE-selected, ultraluminous, high-redshift AGNs(2017) Jones, Suzy F; Blain, Andrew W; Assef, Roberto J; Eisenhardt, Peter; Lonsdale, Carol; Condon, James; Farrah, Duncan; Tsai, Chao-Wei; Bridge, Carrie; Wu, Jingwen; Wright, Edward L; Jarrett, TomWe investigate extremely luminous dusty galaxies in the environments around Wide-field Infrared Survey Explorer (WISE)-selected hot dust-obscured galaxies (Hot DOGs) and WISE/radio-selected active galactic nuclei (AGNs) at average redshifts of z = 2.7 and 1.7, respectively. Previous observations have detected overdensities of companion submillimetre-selected sources around 10 Hot DOGs and 30 WISE/radio AGNs, with overdensities of similar to 2-3 and similar to 5-6, respectively. We find that the space densities in both samples to be overdense compared to normal star-forming galaxies and submillimetre galaxies (SMGs) in the Submillimetre Common-User Bolometer Array 2 (SCUBA-2) Cosmology Legacy Survey (S2CLS). Both samples of companion sources have consistent mid-infrared (mid-IR) colours and mid-IR to submm ratios as SMGs. The brighter population around WISE/radio AGNs could be responsible for the higher overdensity reported. We also find that the star formation rate densities are higher than the field, but consistent with clusters of dusty galaxies. WISE-selected AGNs appear to be good signposts for protoclusters at high redshift on arcmin scales. The results reported here provide an upper limit to the strength of angular clustering using the two-point correlation function. Monte Carlo simulations show no angular correlation, which could indicate protoclusters on scales larger than the SCUBA-2 1.5-arcmin scale maps.

The purpose of this study was to evaluate adherence to the 3 recommendations most strongly associated with health outcomes in new 24-h movement guidelines and their relationship with adiposity (obesity and body mass index z-score) across countries participating in the International Study of Childhood Obesity, Lifestyle and the Environment (ISCOLE). Methods Cross-sectional results were based on 6128 children aged 9–11 years from the 12 countries of ISCOLE. Sleep duration and moderate-to-vigorous physical activity (MVPA) were assessed using accelerometry. Screen time was measured through self-report. Body weight and height were measured. Body mass index (BMI, kg · m −2 ) was calculated, and BMI z-scores were computed using age- and sex-specific reference data from the World Health Organization. Obesity was defined as a BMI z-score > +2 SD. Meeting the overall 24-h movement guidelines was defined as: 9 to 11 h/night of sleep, ≤2 h/day of screen time, and at least 60 min/day of MVPA. Age, sex, highest parental education and unhealthy diet pattern score were included as covariates in statistical models. Associations between meeting vs. not meeting each single recommendation (and combinations) with obesity were assessed with odds ratios calculated using generalized linear mixed models. A linear mixed model was used to examine the differences in BMI z-scores between children meeting vs. not meeting the different combinations of recommendations. Results The global prevalence of children meeting the overall recommendations (all three behaviors) was 7%, with children from Australia and Canada showing the highest adherence (15%). Children meeting the three recommendations had lower odds ratios for obesity compared to those meeting none of the recommendations (OR = 0.28, 95% CI 0.18–0.45). Compared to not meeting the 24-h movement recommendations either independently or combined, meeting them was significantly associated with a lower BMI z-score. Whenever the MVPA recommendation was included in the analysis the odds ratios for obesity were lower. Conclusions For ISCOLE participants meeting these 3 healthy movement recommendations the odds ratios of being obese or having high BMI z-scores were lower. However, only a small percentage of children met all recommendations. Future efforts should aim to find promising ways to increase daily physical activity, reduce screen time, and ensure an adequate night’s sleep in children. - ItemOpen AccessStorm-triggered landslides in the Peruvian Andes and implications for topography, carbon cycles, and biodiversity(2016) West, A J; New, M; Malhi, YIn this study, we assess the geomorphic role of a rare, large-magnitude landslide-triggering event and consider its effect on mountain forest ecosystems and the erosion of organic carbon in an Andean river catchment. Proximal triggers such as large rain storms are known to cause large numbers of landslides, but the relative effects of such low-frequency, high-magnitude events are not well known in the context of more regular, smaller events. We develop a 25-year duration, annual-resolution landslide inventory by mapping landslide occurrence in the Kosñipata Valley, Peru, from 1988 to 2012 using Landsat, QuickBird, and WorldView satellite images. Catchment-wide landslide rates were high, averaging 0.076 % yr−1 by area. As a result, landslides on average completely turn over hillslopes every ∼ 1320 years, although our data suggest that landslide occurrence varies spatially and temporally, such that turnover times are likely to be non-uniform. In total, landslides stripped 26 ± 4 tC km−2 yr−1 of organic carbon from soil (80 %) and vegetation (20 %) during the study period. A single rain storm in March 2010 accounted for 27 % of all landslide area observed during the 25-year study and accounted for 26 % of the landslide-associated organic carbon flux. An approximately linear magnitude–frequency relationship for annual landslide areas suggests that large storms contribute an equivalent landslide failure area to the sum of lower-frequency landslide events occurring over the same period. However, the spatial distribution of landslides associated with the 2010 storm is distinct. On the basis of precipitation statistics and landscape morphology, we hypothesise that focusing of storm-triggered landslide erosion at lower elevations in the Kosñipata catchment may be characteristic of longer-term patterns. These patterns may have implications for the source and composition of sediments and organic material supplied to river systems of the Amazon Basin, and, through focusing of regular ecological disturbance, for the species composition of forested ecosystems in the region.
- ItemOpen AccessThe 2016 super-Eddington outburst of SMC X-3: X-ray and optical properties and system parameters(2017) Townsend, L J; McBride, V A; Buckley, D A H; Evans, P A; Udalski, AOn 2016 July 30 (MJD 57599), observations of the Small Magellanic Cloud by Swift/XRT found an increase in X-ray counts coming from a position consistent with the Be/X-ray binary pulsar SMC X-3. Follow-up observations on 2016 August 3 (MJD 57603) and 2016 August 10 (MJD 57610) revealed a rapidly increasing count rate and confirmed the onset of a new X-ray outburst from the system. Further monitoring by Swift began to uncover the enormity of the outburst, which peaked at 1.2 x 1039 erg/s on 2016 August 25 (MJD 57625). The system then began a gradual decline in flux that was still continuing over 5 months after the initial detection. We explore the X-ray and optical behaviour of SMC X-3 between 2016 July 30 and 2016 December 18 during this super-Eddington outburst. We apply a binary model to the spin-period evolution that takes into account the complex accretion changes over the outburst, to solve for the orbital parameters. Our results show SMC X-3 to be a system with a moderately low eccentricity amongst the Be/X-ray binary systems and to have a dynamically determined orbital period statistically consistent with the prominent period measured in the OGLE optical light curve. Our optical and X-ray derived ephemerides show that the peak in optical flux occurs roughly 6 days after periastron. The measured increase in I-band flux from the counterpart during the outburst is reflected in the measured equivalent width of the H-alpha line emission, though the H-alpha emission itself seems variable on sub-day time-scales, possibly due to the NS interacting with an inhomogeneous disc.

- ItemOpen AccessVaricella zoster virus-associated morbidity and mortality in Africa – a systematic review(2017) Hussey, Hannah; Abdullahi, Leila; Collins, Jamie; Muloiwa, Rudzani; Hussey, Gregory; Kagina, BenjaminBACKGROUND: Varicella zoster virus (VZV) causes varicella and herpes zoster. These vaccine preventable diseases are common globally. Most available data on VZV epidemiology are from industrialised temperate countries and cannot be used to guide decisions on the immunization policy against VZV in Africa. This systematic review aims to review the published data on VZV morbidity and mortality in Africa. METHODS: All published studies conducted in Africa from 1974 to 2015 were eligible. Eligible studies must have reported any VZV epidemiological measure (incidence, prevalence, hospitalization rate and mortality rate). For inclusion in the review, studies must have used a defined VZV case definition, be it clinical or laboratory-based. RESULTS: Twenty articles from 13 African countries were included in the review. Most included studies were cross-sectional, conducted on hospitalized patients, and half of the studies used varying serological methods for diagnosis. VZV seroprevalence was very high among adults. Limited data on VZV seroprevalence in children showed very low seropositivity to anti-VZV antibodies. Co-morbidity with VZV was common. CONCLUSION: There is lack of quality data that could be used to develop VZV control programmes, including vaccination, in Africa. TRIAL REGISTRATION: PROSPERO 2015: CRD42015026144 .
